FRINGE BENEFITS

1.The following insurance will be paid for by the District for both the employee and family:

A.Medical Insurance -

Option 1 - Blue Cross dual option HMO/PPO plan.  The employee's share for the PPO will be $134.00 per month with a $10.00 office visit and a $10.00/$20.00 ($10.00/generic and $20.00/non-generic) co-payment for prescription drugs.  The HMO will have a $5.00 office visit and $5.00/$10.00 co-payment for prescriptions/drugs.  There is no monthly cost to the employee for the HMO.

Option 2 - Kaiser (HMO) with a $10 office visit and $5 prescription/drugs; this plan shall be called the Kaiser Permanente Plan Q.  The employee's cost for Kaiser will be $22.10 per month.

B.MES 12/12/12 plan with a $5.00 deductible.

C.Dental Insurance - REEP dual otion plan ($2,000 maximum without orthondia benefits OR $1,500 maximum with orthodontia benefits)

Any additional increase in health benefit costs shall be subject to the meet and negotiations process.

Parties agree that the cost to the District for the increase and the health benefit contribution to the above programs shall be credited toward the total compensation package offered Yucaipa-Calimesa Educators Association for the 1991-92 and future school year.

2.The following insurance will be paid for by the District for the employee only:

A.Term-life insurance policy of $25,000, evidence of insurability as may be required by the provider.

3.Part-time employees will be provided coverage in accordance with Article 12 - Compensation and Benefits.

4.  The employee may choose to obtain the following insurance coverage from his or her salary:

A.Tax-sheltered annuity

B.Life insurance

Enrollment shall be voluntary and optional to employee or employee and family.  Selection of these various insurance benefits shall be accomplished on the standard District authorization form annually before October 15.

Any employee eligible for coverage under any of the adopted District fringe benefit plans may elect, where applicable, to include any and all dependents for said coverage and authorize that an appropriate deduction from his or her regular salary income be made for the payment of said premiums.

Insurance shall become effective on the first day of the calendar month following submission of the necessary written authorization and its incorporation into the payroll procedure.

The insurance broker will be mutually agreed upon by both the District and the Y-CEA unit.


FRINGE BENEFITS, DISABILITY AND RETIREMENT INSURANCE

1.This agreement is to be effective only for employees who retired or were disabled during or after the 1981-82 school year and is not to affect any prior retired or disabled employee.  For purposes of this section, a regular retired employee shall be at least fifty-five (55) years of age.  This program is effective to age 65 or eligibility for full Medicare benefits including retirement and disability program, whichever comes first.  The benefits noted below shall be in accordance with the insurance program granted to the certificated personnel.

Retiree

The following insurance will be paid for by the District for both the retired employee and one dependent. The employee must have been employed within the District for any combination of 25 years and must have been an employee of the district for at least five consecutive years immediately preceding retirement, or for at least 10 consecutive years immediately preceding retirement.

A.Major medical and health insurance 

B.Prescription insurance

C.The spouse of a deceased retired employee, covered by this provision, shall be allowed to continue in these programs, where permitted by the carrier, through providing the District the appropriate premium at the group rate.

The District will pay for vision insurance for the retired employee only.

The coverage provided for retirees or disabled shall be the same as that provided for employees actively working in the district, including the employer's contribution to the coverage.

Any additional increase in health benefit costs shall be subject to the meet and negotiations process.

Parties agree that the cost to the District for any increase as well as the health benefit contribution to the above programs shall be credited toward the total compensation package offered Yucaipa-Calimesa Educators Association for the 1991-92 and future school year.

For medical coverage beginning in 1991-92, the District shall provide for retirees a three-rate structure, i.e. a rate for families, employee plus one, or single coverage.  The District shall take the difference between the super composite rate applicable to regular employee coverage and the actual coverage provided to the retiree, i.e. retiree and one dependent or single coverage, and deposit these funds in the District's special reserve for retiree/disabled benefits to reduce the outstanding liability for currently retired employees.

Disabled

The following insurance will be paid for by the District provided the employee has been employed within the District for any combination of 25 years and must have been an employee with the district for at least five consecutive years immediately preceding retirement.

A.Major medical and health insurance

B.Prescription insurance

C.Vision insurance

2.Effective only for those employees who retired after the beginning of the 1982-83 school year, the District will pay for vision insurance for one (1) dependent of the retired employee.  For purposes of this section, a regular retired employee shall be at least fifty-five (55) years of age.  This program is not to affect any prior retired employee or disabled employee and is to be effective to age sixty-five (65) or eligibility for full Medicare benefits, whichever comes first.

3.The spouse, not yet eligible for full Medicare benefits, of a retired employee, covered by this provision, age 65 or eligible for full Medicare benefits, shall be allowed to continue in these programs, where permitted by the carrier, through providing the District the appropriate premium at the group rate until the spouse is age 65 or eligible for full Medicare benefits, whichever comes first.

The coverage provided for retirees or disabled shall be the same as that provided for employees actively working in the district, including the employer's contribution to the coverage.

Any additional increase in health benefit costs shall be subject to the meet and negotiations process.

FRINGE BENEFITS, SPOUSE OF DECEASED EMPLOYEE

The spouse of deceased employees who are participants in the District insurance program at the time of the death of the employee shall be allowed to continue in those programs, where permitted by the carrier, through providing the District the appropriate premium at the group rate.

This agreement is to be effective only for members of the unit who are in the employ of the school district beginning the 1982-83 school year.

The request to participate in this program must be received in writing by no later than two (2) weeks subsequent to the death of the employee.

This program is effective to age sixty-five (65) or eligibility for full Medicare benefits.
